  • Law Firm – NRIC of the applicant, Bar Council Malaysia acknowledgement letter.
  • Clinic – NRIC of the applicant, letter from Malaysia Medical Council & Form 12
  • NGO / Others – NRIC of the applicants, acknowledgement letter from the related Ministry/Department
